What's a magic item without a bad pun or two?
Ring of the Powerslav
(Apologies to Iron Maiden)
A cursed item, this ring usually cannot be removed. Its primary effect is to transform the wearer into a caricature of Stalin, complete with megalomaniac tendencies and a penchant for cheap vodka and caviar.

The one I came up with was Schrodinger's Spellbook.
When you open Schrodinger's Spellbook, roll a d100. On a 01, it's blank. On any other result, it contains one randomly generated spell.
Schrodinger's Spellbook contains one spell.
Schrodinger's Spellbook contains every spell ever cast.
Schrodinger's Spellbook contains no spells.
All at once, until you open it up and the wave form collapses.

And of course, on a less lofty plane there are items like the red rubber fireball, the fire bucket of endless water, the red leather teddy of distraction, etc.

And it's not just in magic items. It's actually worse in character names than anywhere else (for me at least), but the tendency toward bad puns shows up pretty much everywhere.

There is a theory which states that if Schrodinger's Spellbook is put in a box with a cat, the universe will instantly be completely destroyed and replaced by something even stranger.
There is another theory which states that this has already happened.

"Bad" puns... do we want to go there? 'Bad' is the word for "bath" in most Germanic languages, isn't it? Often used to denote hot springs? It's one of those words people often don't realize all the meanings of, just like people who consider opera to be pretentious without ever learning that "opera" is the plural of "opus", a word often used as a synonym for "masterwork". So of course it's pretentious. Apparently it's supposed to be.
I'm using Budapest as a template for a city campaign, and looking up its history I learned that it was founded by the Romans who named it "abundance of water" for its springs and baths. So for the fantasy setting I'm using with Budapest as its template, I decided that they call it 'ultimate hot springs', or as it's rendered in the local language....
Bad Opera.
