When you add stat bonus's to skills do you just add the chart bonus's from your stat or do you include racial bonuses as well?

You add in the racial bonuses as well.

Where does it say that?

Use the "total" stat modifier. This is the modifier for the stat score, plus species/racial modifier, plus any other modifiers.
Example 1: If modifier for stat score is +5, species/racial modifier is +3, and an other modifier is +2, then the "total" stat modifier is a +10.
Example 2: If modifier for stat score is +2, species/racial modifier is -1, and an other modifier is -2, then the "total" stat modifier is a -1.
